Description

We present the methods and results from an institution wide rollout of a standardized handoff format across multiple residency programs. This was a QI project that involved co operation from residents across multiple specialty brought together by the House staff quality council and significant support from the Office of Continuing Medical Educations. The project was funded by the hospital's Patient Safety Grant and received backing from the Quality and Safety leadership. Residents were trained in the IPASS handoff format to decrease systemic errors during transitions of care. Quality of handoffs were measured persistently.
