Implement and assess the efficacy of a structured handoff process (I-PASS) on two General Medicine teams as part of the Society of Hospital Medicine-IPASS Mentored Implementation program, given the proven efficacy of the I-PASS handoff bundle in decreasing preventable adverse events. Prior to this initiative, there was no formal training in handoffs or structured format or process for supervision of handoffs, called for by the ACGME Clinical Learning Environment Review.
Baseline data was collected in the form of a baseline survey on house staff perception of handoff quality as well as pre-intervention observations of handoffs. All new interns underwent simulation training in the IPASS format during orientation in June 2015. All PGY2 and PGY3 residents underwent IPASS champion training in July 2015. The goal is for each intern on MP3 and MP4 to have 2 handoffs assessed during their 3 week rotation, using assessment tools for the giver, the receiver and the written handoff document. This will allow assessment of performance on IPASS adoption as well as allow for feedback
Early data shows observations
are occurring and the I-PASS handoff format is being used. Performance will be followed closely to
ensure sustainment. Expansion to other medical teams and other services at BMC will
benefit in bringing about a cultural change that results in a safer transition
of care environment.
