Two lectures from the Maryland Telluride Experience from 2016. To be presented at grand rounds at Metropolitan Hospital 08/2016 as part of our quality improvement education. This lecture deals with realizing that some level of error is built into us all being 'human' and that often the way to improve isnt to expect humans to be perfect, but to rather find ways to assure mistakes are harder to make by human error alone and the system itself is improved.
