Utilizing Standardized Audits to Facilitate Improvement. The need to improve performance in Medication Reconciliation (MR) is well recognized, but several factors can limit success:1) The process lacks a universally accepted definition and process steps2) Improvement is difficult to measure in such a qualitative process3) Variability in methods hinders generalizability across medical centers In this presentation, we demonstrate our tool to standardize the process of evaluating the current state of medication reconciliation at the VA Boston West Roxbury.
